Growth-Related Musculoskeletal Pain

Growth-Related Musculoskeletal Pain (GRMP) refers to aches and discomfort in muscles, joints, or bones that commonly occur in children and adolescents during periods of rapid growth. It is often non-serious and temporary, linked to the body adjusting as bones grow faster than muscles, tendons, and ligaments.
A common example is “growing pains,” which usually affect the legs.

This condition is not caused by disease or injury, but rather by normal developmental changes.

Symptoms of Growth-Related Musculoskeletal Pain

Symptoms can vary, but commonly include:

  • Dull or aching pain in legs, knees, calves, thighs, or arms

  • Pain occurring in the evening or at night, often improving by morning

  • Discomfort affecting both sides of the body

  • Muscle stiffness or tightness

  • Mild swelling or tenderness (without redness or heat)

  • Pain that comes and goes rather than being constant

  • No limitation in normal activities during the day

Causes of Growth-Related Musculoskeletal Pain

The exact cause is not always identifiable, and multiple factors may be involved. Common contributing factors include:

  • Rapid bone growth stretching muscles and tendons

  • Muscle fatigue from physical activities such as running, jumping, or sports

  • Poor posture or biomechanics during growth phases

  • Increased physical activity without adequate rest

  • Reduced flexibility as muscles struggle to keep up with bone lengthening

  • Low pain threshold or sensitivity in some children

When to Seek Medical Advice

Consult a professional if the pain:

  • Is severe or persistent

  • Occurs during the day and limits activity

  • Is accompanied by swelling, redness, fever, or limping

  • Affects only one side consistently

  • Continues beyond typical growth periods

  • Accompanied by other signs, such as fever, severe swelling, limping, rash
